1.  Attach a copy of your certification by EPD.

2.  Attach a copy of your current Erosion and Sediment Control (E&SC) Ordinance with any amendments.

3.  List other ordinances / guidelines which could possibly conflict with your E&SC program.

4.  a.  How many permits were issued last year?  List by category if more than one type was issued.

      b.  How many permits have been issued this year?

5.  Do you maintain a log of plans reviewed / permits issued?  If so, attach a sample.

6.  List those employees that are actively engaged in your E&SC program.  Include in % column the amount of the individual's time devoted strictly to E&SC.
